Insurance & coverage
Most-asked questions about plans we accept, NJ FamilyCare (Medicaid), and how we verify coverage.
Do you accept NJ FamilyCare (Medicaid)?
Yes. We accept all five NJ FamilyCare managed care organizations: Horizon NJ Health, Aetna Better Health of New Jersey, UnitedHealthcare Community Plan of New Jersey, Wellpoint, and FidelisCare. NJ FamilyCare members pay $0 out-of-pocket for therapy, psychiatry, and medication management at our clinic. You'll need your member ID card for your first visit; we verify your eligibility before you arrive.Which Medicaid MCOs are accepted at Positive Reset Eatontown?
All five MCOs that participate in NJ FamilyCare: Horizon NJ Health, Aetna Better Health of New Jersey, UnitedHealthcare Community Plan of New Jersey, Wellpoint (formerly Amerigroup), and FidelisCare New Jersey. If you're not sure which MCO you're enrolled with, the name is printed on the back of your NJ FamilyCare card, or you can call NJ FamilyCare member services at 1-800-701-0710.Do you accept Medicare?
Yes. We accept Original Medicare (Parts A and B) for outpatient mental health, including therapy and psychiatry. We also accept several Medicare Advantage plans — call us at (732) 724-1234 with the name of your specific Medicare Advantage plan and we'll verify in-network status before you book.Do you accept TRICARE?
Yes, we are an in-network TRICARE provider. We see active duty, retirees, and family members covered under TRICARE for mental health care. TRICARE coverage rules differ by plan (Prime, Select, For Life), so we verify your specific plan benefits before your first visit.Do you accept Aetna?
Yes, we accept commercial Aetna plans (HMO, PPO, EPO) for mental health care. Note that Aetna Better Health of New Jersey is a separate Medicaid product — also accepted, but billed differently. If you have Aetna through your employer, that's the commercial plan; if you have Aetna Better Health of New Jersey through NJ FamilyCare, that's the Medicaid product.Do you accept Cigna?
Yes. We accept Cigna commercial plans for mental health care, including most employer-sponsored Cigna plans and Cigna marketplace plans. We verify in-network benefits before your first visit so you know what to expect.Do you accept Horizon Blue Cross Blue Shield?
Yes. We accept Horizon BCBS commercial plans, including OMNIA and most employer plans. Horizon NJ Health (the Medicaid MCO) is a separate product — also accepted, but billed differently. If you have Horizon through your employer, that's commercial; if you have Horizon NJ Health through NJ FamilyCare, that's Medicaid.Do you accept United Healthcare or Oxford?
Yes. We accept commercial UnitedHealthcare and Oxford plans. UHC Community Plan of New Jersey (the Medicaid MCO) is a separate product — also accepted, but billed differently. Tell us which plan you have when you call so we can verify in-network status.Do you accept Oscar?
Yes, we accept Oscar Health for mental health care. Oscar plans purchased through the New Jersey marketplace and through employers are both covered. What if my insurance plan isn't listed?
Call us at (732) 724-1234. We accept several plans not always listed on our website, and we can usually verify a plan within 24 hours. If we're truly out-of-network for your plan, we'll tell you up front and offer a sliding-scale self-pay rate. What does NJ FamilyCare cover for mental health?
NJ FamilyCare covers individual therapy, family therapy, group therapy, psychiatric evaluations, medication management, partial hospitalization, and crisis services. Coverage of telehealth, inpatient care, and substance use treatment varies by MCO and plan tier. Specific session limits and prior-authorization requirements differ by MCO — see the [NJ FamilyCare benefits handbook](https://www.njfamilycare.org) or ask us at intake. Will my child be covered under NJ FamilyCare?
Yes. NJ FamilyCare covers mental health care for children and teens — individual therapy, family therapy, psychiatric evaluations, and medication management. Children's coverage applies in NJ FamilyCare Plan A and Plan B; coverage in Plan C and D differs. Bring the child's NJ FamilyCare card to the first visit. Is there a copay for therapy if I have Medicaid?
No. NJ FamilyCare members pay $0 out-of-pocket for therapy and psychiatry visits at Positive Reset Eatontown. There's no copay, no coinsurance, and no deductible for in-network mental health care. How much does therapy cost if I'm self-pay?
Our self-pay rates are tiered: $150 for individual therapy with a licensed therapist, $200 for psychiatric evaluations, and $125 for medication-management follow-up visits. We offer a sliding-scale rate for visitors who can't afford the full fee — ask at intake. Self-pay rates do not require insurance verification, but we'll always check whether your insurance might cover the visit before billing. What is the No Surprises Act and how does it apply?
The No Surprises Act (CMS) protects you from getting an unexpected bill for in-network care. At Positive Reset Eatontown, we verify your insurance benefits before your first visit and tell you exactly what you'll pay — no surprise bills. If you're a self-pay patient, you'll receive a Good Faith Estimate before treatment begins. See our [No Surprise Billing](/insurance/no-surprise-billing/) page for the full policy. How do I verify my insurance before booking?
Call us at (732) 724-1234. Have your insurance card ready — we'll need the member ID, group number (if any), and the plan name on the front of the card. We verify benefits in 5–10 minutes for most plans, or within 24 hours for plans that require a callback to the insurer.Do I need a referral from my primary care doctor?
For most plans, no. Commercial plans like Aetna, Cigna, Horizon BCBS, Oxford, and UnitedHealthcare typically don't require a referral for outpatient mental health. Some Medicare Advantage plans do. NJ FamilyCare doesn't require a referral for outpatient therapy or psychiatry. We tell you up front if your specific plan needs one. What if my insurance changes between visits?
Tell us as soon as possible. We re-verify benefits when an insurance plan changes — call (732) 724-1234 with the new card. Coverage of mental health care often continues without interruption, but the copay or coverage level may shift, and we want you to know what to expect for the next visit. ---
Costs & billing
What you'll pay, what your plan covers, and our no-surprise-billing promise.
How much will therapy cost me?
If you're insured and we're in-network, you typically pay only your plan's copay or coinsurance — usually $0 to $40 per visit. NJ FamilyCare members pay $0. Self-pay rates start at $125 for medication management and $150 for individual therapy. We tell you the exact cost before your first visit, per the No Surprises Act.Will I get a surprise bill?
No. We verify your insurance benefits before your first visit and tell you what you'll pay. If you're self-pay, you'll receive a Good Faith Estimate before any treatment. We follow the federal No Surprises Act, which protects you from unexpected bills for in-network care. If something on a bill doesn't match what we told you, call us at (732) 724-1234 and we'll fix it.Do you offer a sliding scale?
Yes, for self-pay patients who can't afford our standard rates. Sliding-scale rates are based on household income and need. Ask at intake — we don't advertise the specific rates because they vary by what you can sustainably pay long-term, and we want a real conversation about what's affordable for you, not a bracket on a website.Do you accept HSA or FSA payments?
Yes. We accept Health Savings Account (HSA) and Flexible Spending Account (FSA) cards or reimbursement requests for therapy, psychiatry, and medication management. Both qualify under IRS rules for mental health care. We provide itemized receipts (superbills) for any visit if you need to submit for reimbursement. Can I get a superbill for out-of-network reimbursement?
Yes. If we're out-of-network for your plan and you're self-pay, we provide a superbill on request. The superbill includes the procedure code, diagnosis code, and our tax ID — everything your insurance needs to reimburse you. You submit the superbill to your insurer; reimbursement varies by plan. What forms of payment do you accept?
Credit cards (Visa, Mastercard, American Express, Discover), debit cards, HSA/FSA cards, and ACH bank transfers. We charge the patient-responsibility portion (copay, coinsurance, or self-pay rate) at the time of service. We don't accept cash or paper checks for routine visits. What happens if I can't afford treatment?
Call us at (732) 724-1234 and tell us. We have sliding-scale rates for self-pay, and we can sometimes refer to community mental health centers (CMHCs) that offer free or near-free care for uninsured residents. If you're eligible for NJ FamilyCare and not enrolled, we can help you start the application — coverage often kicks in within 30–45 days. Will my employer or family see my treatment records?
No. Mental health records at Positive Reset Eatontown are protected under HIPAA. Your employer, family members (other than parents of minors with consent), and other parties cannot see your records without your written permission. Insurance billing only shows that a session occurred and the diagnosis code — not the content of the session. ---
Scheduling & first visit
How to book, where we are, what to expect, and what to bring on your first visit.
How long until I can be seen?
Most new patients are seen within 7–14 days of their first call. Telehealth visits often have shorter wait times than in-person. If you're in crisis, call us — we triage same-week appointments for active safety concerns where appropriate, or refer you to crisis resources if our outpatient setting isn't the right level of care.Where are you located?
615 Hope Road, Suite 3B, Eatontown, NJ 07724. We're in the Hope Road professional building, with parking on-site and accessible by NJ Transit bus routes. We see patients in-person at this location, and we offer telehealth statewide across New Jersey for visitors anywhere in the state. What are your hours?
Monday through Thursday: 9am to 8pm. Friday: 9am to 6pm. Saturday: by appointment for established patients. We're closed Sundays and major holidays. Telehealth visits follow the same hours and are available across all New Jersey time-zone-equivalent business hours. How do I book my first appointment?
Call (732) 724-1234, or use the booking form on this site. The first call takes 10–15 minutes — we verify insurance, ask about your concern, and match you with the right clinician. We confirm your first appointment by phone or email within 24 business hours. Can I book online?
You can request a first appointment using the form on any page of this site. We'll call back within 24 business hours to confirm a time, verify insurance, and set you up with the right clinician. For now, we don't offer fully self-service online scheduling — the first call lets us match you well. What should I bring to my first visit?
Your insurance card, a photo ID, and a list of any medications you're currently taking (including dose). If you're transferring care, recent treatment records or a discharge summary helps. For psychiatric evaluations, also bring any prior psychiatric records and a list of medications you've tried before. The first visit is usually 60–90 minutes.Do you have evening or weekend appointments?
Yes. We see patients evenings until 8pm Monday through Thursday and have Saturday appointments by request for established patients. Telehealth often has more flexible evening availability than in-person. Tell us at intake what time of day works best and we'll match you to a clinician with that availability.Do you offer telehealth visits?
Yes. We offer secure video therapy and psychiatry across all of New Jersey. Telehealth visits are billed the same as in-person and covered the same by most insurance plans. You'll need a private space, a stable internet connection, and a device with a camera. We send a secure link before your first session. What languages do you offer therapy in?
English is our primary language. We have clinicians who also offer therapy in Spanish, and limited availability in other languages — call us at (732) 724-1234 to ask about a specific language and we'll match you if possible. For other languages, we can sometimes coordinate with a certified medical interpreter. Is your office accessible?
Yes. The 615 Hope Road building is wheelchair accessible with elevator access to Suite 3B. Parking is available directly outside the building. If you need any specific accommodations — large-print intake forms, a fragrance-free room, support animal accommodations — tell us at intake. What if I miss an appointment?
We charge a $50 no-show fee for missed appointments without 24 hours notice. The fee does not apply to insurance-covered visits — only to patient-pay portions. Two no-shows in a row is usually a sign that scheduling needs to change; we'll work with you to find a time that's actually realistic. Can I bring someone with me to my first visit?
Yes. Many people bring a partner, family member, or friend to the first visit, especially for evaluations. The clinician will ask whether you want them in the session itself or in the waiting room — either is fine. For minors, a parent or guardian must be present to sign consent at the first visit. ---
Conditions & treatment
Common questions about anxiety, depression, ADHD, PTSD, postpartum, teen mental health, and the treatments that work.
What conditions do you treat at Positive Reset Eatontown?
We treat a full range of outpatient mental health conditions: anxiety disorders, depression (including treatment-resistant), bipolar disorder, ADHD, OCD, PTSD, postpartum depression and perinatal mood disorders, eating disorders, substance use disorders alongside mental health (dual diagnosis), and adjustment-related issues. We see adults, adolescents (13+), couples, and families. We do not provide inpatient or 24/7 crisis care.Do you see teens?
Yes. We see adolescents ages 13 and older for individual therapy, family therapy, psychiatric evaluations, and medication management. We have clinicians who specialize in adolescent care, including teen anxiety, teen depression, teen ADHD, and family conflict. Most parents call to set up the first visit, but teens 14+ can usually book directly on their own with parental consent on file.Do you treat ADHD?
Yes. We treat ADHD in adults, teens (13+), and children when paired with family therapy. Treatment includes diagnostic evaluation, therapy (especially CBT and skills-based work), and medication management. We can assess and prescribe stimulant and non-stimulant medications. Our typical evaluation involves a 90-minute initial session plus standardized rating scales (e.g., the Conners or BASC).Do you offer TMS therapy?
Yes. We offer Transcranial Magnetic Stimulation (TMS) for adults with treatment-resistant depression — meaning depression that hasn't responded to at least two adequate trials of antidepressant medication. A typical TMS course is 5 days a week for 4–6 weeks. Most insurance plans cover TMS for treatment-resistant depression with prior authorization; we handle that paperwork. What is the difference between a therapist and a psychiatrist?
A therapist (LCSW, LPC, LMFT, PhD, PsyD) provides talk therapy — sessions where you work through thoughts, feelings, and behaviors with evidence-based methods like CBT, DBT, or EMDR. Therapists do not prescribe medication. A psychiatrist (MD, DO) is a medical doctor who specializes in mental health — they evaluate, prescribe, and manage psychiatric medications. At Positive Reset Eatontown, both work together when needed.Do I need therapy, medication, or both?
It depends on the condition and your preferences. Mild-to-moderate anxiety and depression often respond well to therapy alone. Severe depression, bipolar disorder, severe OCD, and ADHD typically require medication, sometimes alone but more often combined with therapy. Treatment-resistant depression may benefit from TMS. We make this recommendation after the first 1–2 sessions, and you always have the final say.How long does treatment usually take?
For most outpatient mental health concerns, therapy runs 8–20 sessions over 3–6 months. Some people stay longer for ongoing support; others finish in fewer sessions. Medication is more variable — some people take an antidepressant for 6–12 months, others longer. We re-evaluate the plan every 90 days so you're not paying for sessions you don't need. What therapy modalities do your clinicians use?
We use evidence-based modalities: Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Eye Movement Desensitization and Reprocessing (EMDR), Acceptance and Commitment Therapy (ACT), Interpersonal Therapy (IPT), Behavioral Activation, and Emotionally Focused Therapy (EFT) for couples. Each clinician has primary modalities they're trained in — we match you to a clinician based on your concern and preferences.Do you treat substance use disorders?
We treat substance use disorders when they co-occur with mental health conditions (dual diagnosis), and we have clinicians dual-licensed as LCADC (Licensed Clinical Alcohol and Drug Counselor). We do not provide medical detox, residential treatment, or methadone. For severe substance use, we'll coordinate with a higher level of care and resume outpatient work when appropriate. Can I switch therapists if it's not a good fit?
Yes. Therapeutic fit matters more than credentials, and we make switching easy — call our intake team and we'll match you with a different clinician. About 1 in 6 first matches don't click; this is normal, and switching does not delay treatment by more than a week or two.Do you do online therapy or telehealth?
Yes. We offer telehealth (video) therapy and psychiatry across all of New Jersey. Telehealth works for most outpatient mental health concerns; some psychiatric evaluations and most TMS sessions require in-person visits. Insurance generally covers telehealth at the same rate as in-person care. If you live anywhere in New Jersey, we can see you online — see [telehealth in New Jersey](/services/telehealth-new-jersey/).Will you prescribe controlled substances like Adderall or Xanax?
We can. Stimulants for ADHD (Adderall, Vyvanse, Concerta) are commonly prescribed by our psychiatrists with appropriate diagnosis and monitoring. Benzodiazepines (Xanax, Ativan, Klonopin) are prescribed cautiously and usually short-term, because they carry dependence risk. We follow standard clinical guidelines: we'll never refuse a medication that's clinically indicated, and we won't prescribe one that isn't. ---
Our clinicians
How we hire, who reviews our content, and how to find the right fit for what you're bringing in.
Who are your clinicians?
Our clinical team includes 75+ licensed mental health professionals: therapists (LCSWs, LPCs, LMFTs, PhDs, PsyDs), psychiatrists (MDs, DOs), advanced practice nurses (APNs), and addiction counselors (LCADCs). Each clinician is licensed in New Jersey, credentialed through our practice, and clinically supervised. See our full [team page](/team/) for clinician profiles. How do I choose a therapist?
You don't have to. When you call, our intake team matches you to a clinician based on three things: your concern (anxiety, depression, ADHD, etc.), your insurance and availability, and any preferences you share (gender, age range, modality). Most patients trust the intake match — and if it doesn't click after the first session, switching is easy. Are your clinicians licensed in New Jersey?
Yes. Every clinician at Positive Reset Eatontown holds an active New Jersey license in their discipline (LCSW, LPC, LMFT, MD, DO, APN, etc.). Our practice is licensed by the New Jersey Department of Health, accredited by the Joint Commission, and certified by Medicare. License numbers are public — verify at the [NJ Division of Consumer Affairs](https://www.njconsumeraffairs.gov/).Can I see a specific clinician I read about?
Yes, when their schedule allows. Tell us the clinician's name when you call and we'll check availability — usually 1–4 weeks for a specific clinician versus 7 days for first-available. If your preferred clinician has a longer wait, we'll offer to schedule with them and start care sooner with a different clinician for bridge support. What credentials should I look for in a therapist?
Any of these are appropriate for outpatient therapy in New Jersey: LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor), LMFT (Licensed Marriage and Family Therapist), PsyD (Doctor of Psychology), PhD (clinical psychologist). For medication, look for MD, DO, or APN with psychiatric specialty. Master's-level clinicians under supervision (LSW, LAC, LAMFT) provide good care with appropriate oversight. ---
Crisis resources
If you’re in crisis
If you or someone you love is in immediate danger, call or text 988, or go to the nearest emergency room. The 988 Suicide & Crisis Lifeline is available 24/7. Positive Reset is an outpatient clinic and is not a 24/7 crisis line.
New Jersey residents can also reach the NJ Hopeline at 1-855-654-6735, a state-funded peer-support service for emotional distress.
What should I do if I'm in crisis right now?
Call or text 988 — the national Suicide & Crisis Lifeline. It's free, available 24/7, and staffed by trained counselors. If you're in immediate danger, call 911 or go to your nearest emergency room. Positive Reset Eatontown is an outpatient clinic — we don't provide 24/7 crisis services, but we'll see you within a week for follow-up care once you're safe.Do you have crisis services?
Not 24/7. Positive Reset Eatontown is an outpatient mental health clinic with weekday and Saturday-by-appointment hours. For active crisis (thoughts of suicide, harm to self or others, severe symptoms requiring immediate evaluation), call 988 or go to your nearest emergency room. We can see you for follow-up outpatient care, often within 7 days of a crisis discharge. What if I'm having thoughts of suicide?
Call or text 988 right now. Talk to someone trained to help. If you're in immediate danger, call 911. Once you're safe, call us at (732) 724-1234 and we'll set up an outpatient follow-up — usually within a week. We work closely with hospital emergency departments and partial hospitalization programs across Monmouth and Middlesex counties for transitions of care. What's the difference between a crisis center and outpatient therapy?
A crisis center provides immediate, short-term help during an acute mental health emergency — they can stabilize you, help with hospitalization decisions, and connect you to longer-term care. Outpatient therapy (what we offer) is regular, scheduled sessions over weeks and months for non-crisis treatment. Most people need both at different points: crisis services when symptoms are acute, outpatient when symptoms are manageable. Will my therapist hospitalize me if I tell them I'm struggling?
Almost never. Hospitalization is reserved for active danger to yourself or others, or when you can't safely care for yourself. Therapists are trained to assess risk and partner with you on a safety plan. Telling your therapist you're struggling is exactly what therapy is for, and it almost always leads to a stronger plan, not a hospitalization. ---
Quick answers
Short answers for the questions that come up in the lookup card and on the homepage.
Is your clinic accepting new patients?
Yes. We accept new patients across all conditions and insurance plans we accept. New-patient wait time is 7 days on average. Call (732) 724-1234 or use our booking form. Are you part of Positive Reset Services?
Yes. Positive Reset Eatontown is part of the Positive Reset Services franchise, which includes sister clinics in Manalapan, Freehold, Iselin, Elizabeth, Warren, and East Brunswick. Each clinic is independently licensed and credentialed; clinicians do not see patients across locations except for telehealth across New Jersey.Do you treat patients outside New Jersey?
Generally no. Our clinicians are licensed in New Jersey, which means we can see patients physically located anywhere in New Jersey, but not patients located in other states (even by telehealth). If you're moving and want to continue care across state lines, ask us at intake — sometimes a brief transition is possible during temporary out-of-state stays.Do I have to have a diagnosis to start therapy?
No. You can start therapy with any concern — relationship issues, work stress, life transitions, grief, family conflict — without a formal diagnosis. Most insurance plans require a diagnosis code on the bill for reimbursement, which the clinician assigns based on your presenting concerns. The diagnosis is part of the medical record but not a label you have to share or accept. ---
